To manually submit the required documents, follow these steps:

1.  Download and fill out the necessary forms provided.
2.  Save the completed documents on your device.
3.  You have two options for submitting the paperwork:

a. Send an email with the completed documents attached to [docs@gracehealthservice.com],

or

b. Return to this webpage and click the "Email Completed Paperwork" button to upload and submit the documents.

Please make sure to add your full name and date of birth. 

Complete this only if you don't have health insurance.
